Terms and Conditions for Survey Participation and Prize Draw

1. Introduction

• These terms and conditions apply to the Survey conducted by Meal Ticket ("Company"). By participating in the Survey and entering the prize draw, you agree to be bound by these terms.

2. Eligibility

• Participants must be at least 18 years old.
• Employees of Meal Ticket and their immediate family members are not eligible to participate.

3. Survey Participation

• The Survey begins on May 31, 2024 and ends on July 15, 2024.
• No purchase is necessary to enter or win.

4. Prize Draw Entry

• Upon completing the Survey, participants will have the option to enter the prize draw by providing a valid email address.
• Only one entry per person is permitted.

5. Prizes

• The prize draw includes 1 Visa Gift Card valued at $250.
• Prize is non-transferable and cannot be exchanged for cash.

6. Winner Selection

• Winner will be selected randomly from all eligible entries after the Survey closes.
• Winner will be notified via email within 5 days of the draw.

7. Claiming Prizes

• Winner must claim their prize within 7 days of notification.
• Unclaimed prizes may be forfeited or awarded to an alternate winner.

8. Privacy

• Personal information collected during the Survey will be used solely for the purposes of administering the Survey and prize draw.

9. Liability

• The Company is not responsible for entries not received due to technical issues or any other factors beyond its control.

10.  Changes to Terms.

• The Company reserves the right to amend these terms and conditions without prior notice.
